As the healthcare landscape evolves and the population ages, the demand for innovative care solutions has intensified. Emplify Health, a not-for-profit regional health system, and Homecare Hub, a platform specializing in home care and housing coordination, have announced a partnership to transform patient transition care. This collaboration introduces the Transitional Placement Services (TPS) model, aiming to bridge the gap in patient care during the critical transition from hospital to community settings.
The TPS model is a response to the 'silver tsunami,' the demographic shift as baby boomers age, increasing the need for long-term care services. With a shortage of long-term care facilities, hospitals face significant challenges in discharging patients, leading to extended hospital stays and financial strain. The TPS model, developed by Emplify Health's innovation investment team, Edge, and implemented with Homecare Hub, seeks to alleviate these pressures by ensuring patients receive appropriate care post-discharge.
Launching in fall 2025, the TPS model will initially pilot in La Crosse, Wisconsin, with six dedicated beds in a Homecare Hub facility. This model emphasizes a personalized care plan developed by a multidisciplinary team, leveraging Homecare Hub's technology to connect patients with suitable care options. The initiative promises enhanced patient care, reduced hospital staff burden, and increased cost efficiency, with potential savings of hundreds of thousands of dollars.
The partnership between Emplify Health and Homecare Hub represents a significant advancement in post-acute care, offering a scalable solution to a nationwide challenge.
